Service Project Approval and Attendance Form

 

In order to facilitate and track individual service projects, the form was developed for the Scoutmaster's approval and to track the scout's attendance. This form should be used for all individual service projects that are not Troop organized. All individual service projects must have prior Scoutmaster and/or Troop Committee approval.

 

  1. Fill out Section A and turnit in to the Scoutmaster, Advancement Coordinator or any Adult Leader for approval prior to doing the service project.
  2. Section B should be completed and signed by a member of the Service Project Organization.
  3. Return the form to the Scoutmaster, Advancement Coordinator or any Adult Leader for recording.
